Questions Trail Death of Retired Ogun Director Amid Alleged Investigative Lapses

Fresh concerns have emerged over the death of Mrs. Modupe Edun, a retired director in the Ogun State Civil Service, following the circulation of a preliminary independent investigation report highlighting alleged inconsistencies and suspicious circumstances surrounding the incident.

Mrs. Edun reportedly died at her residence in Grace Haven Estate, Kobape, Ogun State, in circumstances initially described as resulting from a bathroom fall.

However, the independent report dated May 23, 2026, identified several unresolved issues said to require urgent forensic and law enforcement scrutiny.

Among the concerns raised were the delayed notification of relatives, alleged movement and handling of the deceased’s body before security agencies were contacted, visible injuries reportedly observed on the deceased, possible tampering with the scene, and inconsistencies in the timeline surrounding the incident.

The report particularly questioned why no immediate alarm was allegedly raised and why law enforcement authorities were not promptly contacted after the deceased was reportedly discovered in a pool of blood.

It further alleged that the scene was cleaned and the body moved to a mortuary before the formal involvement of the police.

Additional concerns were raised over reports that the mortuary at the Neuropsychiatric Hospital, Aro, Abeokuta, allegedly received the body without complete documentation or formal police referral at the point of deposit.

Observers noted that such procedural lapses, if established, could undermine transparency in death investigations and potentially compromise the integrity of forensic inquiries.

The report also expressed concern over what it described as a slow investigative response by relevant authorities and the apparent absence, so far, of visible indicators of a detailed forensic or criminal investigation into the matter.

According to the findings, the nature and distribution of injuries allegedly observed on the deceased appeared inconsistent with what was initially presented as a straightforward accidental bathroom fall.

The report consequently recommended an immediate forensic review and a comprehensive criminal investigation to determine the actual cause and circumstances surrounding the death.

It also called for the preservation and examination of all relevant evidence, including medical records, photographs, mortuary documentation, and witness statements.

Concerned stakeholders have therefore urged the Ogun State Attorney-General and Commissioner for Justice, the Commissioner of Police, Ogun State Command, and the Director of the Department of State Services (DSS) in the state to ensure a thorough, transparent, and impartial investigation into the incident.

The calls include demands for forensic examination, reconstruction of the scene, verification of timelines, and interrogation of all individuals connected to the matter.

The report concluded that only a comprehensive and professional investigation would help establish the true circumstances surrounding the death and sustain public confidence in the justice process.
